<p><strong>Job Summary</strong></p><ul><li>The Home Care Nurse provides high-quality nursing care to clients in their homes, ensuring their medical needs are met while promoting independence and well-being.</li><li>The role involves traveling to clients’ homes, assessing their health conditions, administering treatments, providing education, and collaborating with healthcare professionals and family members to ensure comprehensive care.</li></ul><p><strong>Key Responsibilities</strong><br />Patient Care & Nursing Services:</p><ul><li>Provide personalized, one-on-one nursing care to clients in their homes, ensuring adherence to care plans developed by healthcare providers.</li><li>Administer medications, treatments, and other medical interventions, following physician orders and nursing protocols.</li><li>Conduct regular assessments of clients' health status, including vital signs, physical conditions, and responses to treatments.</li><li>Perform wound care, catheter care, injections, IV therapy, and other clinical procedures as required.</li></ul><p>Health Monitoring & Documentation:</p><ul><li>Monitor changes in clients’ health and report any significant changes to the supervising nurse or physician.</li><li>Maintain accurate, up-to-date clinical documentation, including assessments, treatment notes, and medication administration records.</li><li>Ensure compliance with health regulations and care home policies regarding record-keeping and reporting.</li></ul><p>Care Coordination & Family Education:</p><ul><li>Collaborate with other healthcare providers, such as doctors, physical therapists, and social workers, to develop and adjust care plans.</li><li>Educate clients and their families about managing chronic conditions, medications, and home safety measures.</li><li>Provide emotional support to clients and families, helping them navigate healthcare challenges and fostering a positive caregiving environment.</li></ul><p>Emergency Response & Critical Care:</p><ul><li>Respond promptly to emergencies and provide appropriate medical interventions, including first aid or emergency procedures.</li><li>Coordinate with hospitals or emergency services when a higher level of care is needed.</li></ul><p>Health Education & Preventive Care:</p><ul><li>Provide health education to clients and their families, emphasizing preventive care, proper nutrition, and healthy living habits.</li><li>Encourage adherence to prescribed treatment plans, including medication schedules, rehabilitation exercises, and follow-up appointments.</li></ul><p>Quality & Compliance:</p><ul><li>Ensure all nursing activities are conducted in compliance with the care home’s policies, healthcare regulations, and best practices.</li><li>Participate in quality assurance programs and contribute to continuous improvement initiatives aimed at enhancing patient care.</li></ul><p><strong>Qualifications & Experience</strong></p><ul><li>Bachelor’s Degree in Nursing B.Sc. Nursing or equivalent nursing qualification.</li><li>Valid nursing license and registration with the relevant nursing council.</li><li>Minimum of 5 years of nursing experience, ideally in home care, geriatrics, community nursing, or related fields.</li><li>Valid driver’s license is required.</li><li>Experience in wound care, medication management, and home-based medical care is an advantage.</li><li>CPR and First Aid certifications are desirable.</li></ul><p>Key Competencies:</p><ul><li>Strong clinical and diagnostic skills, with the ability to work independently in a home setting.</li><li>Excellent communication and interpersonal skills, with a compassionate approach to patient care.</li><li>Organizational skills to manage multiple home visits and maintain accurate records.</li><li>Ability to handle medical emergencies calmly and efficiently.</li><li>Knowledge of home care nursing protocols and best practices.</li><li>Familiarity with home care technology and mobile healthcare tools e.g., remote monitoring devices.</li></ul>
