<p><strong>Job Summary</strong></p><ul><li>The Home Care Assistant provides essential support to clients in their homes, helping them with personal care, daily living activities, and basic healthcare needs.</li><li>The role involves traveling to clients&rsquo; homes, assisting with mobility, personal hygiene, medication reminders, and working under the supervision of healthcare professionals.</li></ul><p><strong>Key Responsibilities</strong><br />Personal Care &amp; Daily Living Support:</p><ul><li>Assist clients with personal hygiene tasks, including bathing, dressing, grooming, and toileting.</li><li>Help clients with mobility, including transferring from bed to wheelchair, assisting with walking, and performing physical activities as needed.</li><li>Provide support with eating, meal preparation, and feeding, if necessary, while following any special dietary requirements.</li></ul><p>Health Monitoring &amp; Basic Care:</p><ul><li>Assist with monitoring clients&rsquo; general health and well-being, reporting any concerns or changes in condition to the supervising nurse.</li><li>Remind clients to take prescribed medications as per care plans and ensure that they follow healthcare routines.</li><li>Support clients with exercises or rehabilitation activities as advised by healthcare professionals.</li><li>Assist with simple wound care and ensure proper care of medical equipment like walkers, oxygen tanks, or other aids.</li></ul><p>Emotional &amp; Social Support:</p><ul><li>Provide companionship and emotional support to clients, helping them stay engaged and socially active.</li><li>Assist with social or recreational activities to improve the quality of life for clients and reduce feelings of isolation.</li><li>Support clients in communicating with their family members and other healthcare professionals.</li></ul><p>Record Keeping &amp; Reporting:</p><ul><li>Maintain accurate daily records of care provided, including tasks performed, observations of the client&rsquo;s health, and any changes in condition.</li><li>Report significant health or safety concerns to the Home Care Supervisor or healthcare team immediately.</li><li>Participate in regular care reviews and provide feedback to help improve the client&rsquo;s care plan.</li></ul><p>Compliance &amp; Professionalism:</p><ul><li>Ensure that all care activities comply with local health regulations, safety standards, and the care home&rsquo;s policies.</li><li>Always respect the privacy and confidentiality of clients, adhering to client confidentiality and data protection standards.</li><li>Attend required training and professional development programs to stay updated on best practices in home care.</li></ul><p><strong>Qualifications &amp; Experience</strong></p><ul><li>Certificate in Nursing Assistance, Caregiving, or a related field is an advantage.</li><li>Minimum of 1 year of experience in nursing, healthcare assistance, or caregiving.</li><li>Valid driver&rsquo;s license is required.</li><li>Experience working with elderly or disabled individuals in a home care setting is preferred.</li><li>Basic understanding of healthcare practices and personal care support.</li><li>CPR and First Aid certification is a plus.</li></ul><p>Key Competencies:</p><ul><li>Compassionate, patient-centered approach to care.</li><li>Good interpersonal and communication skills.</li><li>Ability to handle physical tasks such as lifting, transferring, and assisting clients with mobility.</li><li>Strong organizational skills and the ability to manage multiple clients&rsquo; care needs.</li><li>Ability to work independently with minimal supervision.</li><li>Reliable, punctual, and professional demeanor.</li></ul>
